0724 Objectively Regular Sleep of Optimal Duration and Mortality: The Multi-Ethnic Study of Atherosclerosis

Abstract Introduction Irregularly timed sleep, inconsistent duration, and non-optimal sleep duration have been implicated in the etiology of cardiovascular disease premature mortality. These dimensions map behaviorally to regular bed wake times. Our aim was estimate association optimal with all-cause mortality a diverse US sample. Methods We used data from Multi-Ethnic Study Atherosclerosis Sleep (2010-2013) at Exam 5. measured by 7-day wrist actigraphy, concurrent at-home polysomnography validated questionnaires. K-means clustering on wrist-actigraphy regularity assign individuals categories ‘regular-optimal’ or ‘irregular-insufficient’ labels describing tendencies each group. Using proportional hazard models multi-variate adjustment for socio-demographics (age, female sex, race/ethnicity, marital status, day vs non-day work schedules), lifestyle (moderate-vigorous physical activity, smoke status), health status (depressive symptoms, body mass index, total medications, prevalent disease), measures major disorders (the Apnea-Hypopnea Index, insomnia severity, restless legs), we estimated associations patterns. Sensitivity analyses additionally adjusted or, via propensity scores, matched income, education, chronic lung disease, cancer, hypertension, diabetes, diet. Results Among 1,726 participants who were followed median 7.0 years (Q1-Q3, 6.4-7.4 years,) there 171 deaths. The group had 42% lower risk compared (Hazard Ratio [95% Confidence Interval]: 0.58 [0.43, 0.80]), fully model robust sensitivity analyses. Conclusion Individuals characterized pattern ‘regular optimal’ significantly those an ‘irregular less sufficient’ pattern. phenotype maps times, suggesting potential benefits adherence recommended practices.
